<title>AMA Seeks Feedback on the WISeR Model</title>

Wagner MD, represented IPSIS members at the April 2026, American Medical Association (AMA) Relative Value Update Committee (RUC) meeting in Chicago, Illinois.<br /><br />RUC is an advisory body to the Centers for Medicare and Medicaid Services (CMS) that reviews medical procedures and provides Relative Value Unit (RVU) recommendations. The recommendations approved at this meeting will be presented for final authorization and publication in the next Medicare Physician Fee Schedule.<br /><br /></p>

<description><![CDATA[<p><strong></strong>IPSIS urges you to <a href="https://portal.cms.gov/portal/" target="_blank">review</a> what industry reported about you before the U.S. Centers for Medicare and Medicaid Services (CMS) publishes its 2025 Open Payments <a href="https://www.cms.gov/OpenPayments/Program-Participants/Covered-Recipients/Review-and-Dispute" target="_blank">data</a>.</p><p><strong>Disputes must be initiated by May 15, 2026,</strong> to be reflected in the June 1, 2026, data publication.&nbsp;<br /><br />The CMS website displays records of “transfers of value” from industry, including items over $11 or more than $100 annually in smaller gifts. <title>US-Guided TPIs: Preserving Access</title>

<description><![CDATA[<p><strong><img alt="" src="https://www.ipsismed.org/resource/resmgr/advocacy/26/04-trigger-s.jpg" style="width: 95%; height: 95%;" /><br /></strong><br />IPSIS urges Medicare Administrative Contractors (MACs), First Coast Service Options and Novitas, to revise proposed Local Coverage Determinations (LCDs) that misclassify ultrasound guidance for trigger point injections (TPIs) as investigational.<br /><br />Key concerns:<br /></p><ul><li><strong>PATIENT SAFETY:</strong> Ultrasound guidance is important in higher-risk anatomic regions near nerves, vessels, lungs, or other structures.</li></ul><ul><li><strong>PROCEDURAL ACCURACY: </strong>It may improve accuracy, especially in deeper or more complex muscle groups.</li></ul><p>IPSIS rallied the Multisociety Pain Workgroup (MPW) to coordinate and submit comment letters to <a href="https://www.ipsismed.org/resource/resmgr/advocacy/26/04-TPI-FCSO.pdf" target="_blank">First Coast Service Options</a> and <a href="https://www.ipsismed.org/resource/resmgr/advocacy/26/04-TPI-Novitas.pdf" target="_blank">Novitas</a>, leveraging the collective influence of participating specialty societies.</p>

<title>Lawmakers Push Back on PNB Cuts</title>

<description><![CDATA[<p><strong><img alt="" src="https://www.ipsismed.org/resource/resmgr/advocacy/26/03-PNB-s.jpg" style="width: 95%; height: 95%;" /><br /></strong><strong><br /></strong>IPSIS welcomes <a href="https://www.ipsismed.org/resource/resmgr/advocacy/26/03-PNB.pdf" target="_blank">bipartisan action</a> from 25 lawmakers urging the Centers for Medicare and Medicaid Services (CMS) to withdraw proposed Medicare policies restricting coverage of <strong>peripheral nerve blocks</strong> (PNBs). These lawmakers, representing
    nearly 19 million Americans or about 6% of the U.S. population, are calling on CMS to protect access to safe, effective non-opioid pain treatments as concerns about the proposals continue to grow.<br /><br />IPSIS, alongside the Pain Medicine Coalition
    (PMC) — including ASA, ASRA Pain Medicine, and AAPM&amp;R, <a href="https://www.ipsismed.org/news/720902/Take-5-Minutes-to-Save-PNB-Coverage.htm" target="_blank">strongly opposes</a> these changes.<br /></p>
<p>Key concerns:<br /></p>
<ul>
    <li><strong>Reduced patient access</strong>: Coverage limits would create barriers for Medicare beneficiaries with chronic pain.</li>
    <li><strong>Evidence-based care at risk</strong>: PNBs are a long-established, essential part of pain management.</li>
    <li><strong>Unintended consequences</strong>: Restrictions may push patients toward less appropriate or higher-risk treatments, including opioids.<br /></li>
</ul>
<p>IPSIS and the PMC <a href="https://www.ipsismed.org/news/714257/IPSIS-Takes-Action-to-Safeguard-Patient-Access.htm" target="_blank">urge CMS and Medicare Administrative Contractors</a> (MACs) to withdraw these proposals and protect access to essential
    non-opioid pain care.</p>

<title>Preserving BVNA Access</title>

<description><![CDATA[<p><strong><img alt="" src="https://www.ipsismed.org/resource/resmgr/advocacy/26/03-BVNA-s.jpg" style="width: 95%; height: 95%;" /><br /></strong><strong><br /></strong>IPSIS reinforced its national leadership in interventional pain policy by convening the Multisociety Pain Workgroup (MPW) to submit a unified response to National Government Services (NGS) on its proposed LCD for Basivertebral Nerve Ablation (BVNA).<br /><br />The <a href="https://www.ipsismed.org/resource/resmgr/advocacy/26/03-BVNA.pdf" target="_blank">MPW letter</a> strongly supported Medicare coverage of BVNA and offered evidence-based recommendations to ensure appropriate access and high standards for patient selection. Key recommendations included:<br /></p><ul><li>clarifying psychological screening requirements,</li><li>allowing flexibility in imaging when MRI is not feasible, emphasizing clinical context alongside imaging findings, and</li><li>refining osteoporosis and exclusion criteria to reflect current evidence.</li></ul>

    leading this bipartisan congressional letter to protect patient access. Congressional offices pay close attention to outreach from physicians in their districts — your voice carries significant weight.<br /><br />This legislative grassroots effort
    is in partnership with other pain societies: ASA, ASRA, and AAPM&amp;R.<br /><br />IPSIS has <a href="https://www.ipsismed.org/news/714257/IPSIS-Takes-Action-to-Safeguard-Patient-Access.htm" target="_blank">submitted detailed comments</a> reinforcing
    the clinical evidence, prevailing practice standards, and serious patient access concerns. These concerns were further elevated when the AMA adopted a House of Delegates resolution—co-authored and supported by IPSIS and several national and state
    medical societies—calling on CMS to withdraw or delay the proposed coverage-denying plans.<br /><br />Please act today to protect Medicare patients’ access to PNBs.<br /><br />If finalized, these policies would significantly restrict access for Medicare

    of Delegates (HOD) resolution co-authored and <a href="https://cdn.ymaws.com/www.ipsismed.org/resource/resmgr/advocacy/25/12-PNB-LateResolution.pdf" target="_blank">supported by IPSIS</a>, alongside several national and state medical societies.<br
    /><br />The <a href="https://cdn.ymaws.com/www.ipsismed.org/resource/resmgr/advocacy/25/12-PNB-Resolution830.pdf" target="_blank">resolution</a> urged the AMA to oppose the draft LCDs and to advocate for preserving patient access to evidence-based,
    non-opioid interventional pain care.<br /><br />In its letter to CMS, the AMA emphasized that peripheral nerve blocks are well-established interventions that improve pain, function, and quality of life and play a critical role in comprehensive, patient-centered
    pain management.<br /><br />IPSIS continues to drive a major <a href="https://www.ipsismed.org/news/714257/IPSIS-Takes-Action-to-Safeguard-Patient-Access.htm" target="_blank">advocacy effort on this issue</a> and previously submitted detailed comments
    on the proposed LCDs, reinforcing the clinical evidence, prevailing practice standards, and patient-access concerns that have now been elevated through the AMA process.</p>

    (PNBs), peripheral nerve radiofrequency ablation (RFA), and related interventional procedures used to treat chronic pain.<br /><br />These joint letters, developed through IPSIS’s leadership in collaboration with the <a href="https://www.ipsismed.org/page/MPW" target="_blank">Multisociety Pain Workgroup</a> (MPW) and other supporting organizations, emphasizes that these proposed Local Coverage Determinations (LCDs) fail to reflect the substantial body of clinical evidence supporting the safety, effectiveness,
    and value of these minimally invasive, non-opioid procedures.<br /><br />Click on the MAC names above to read the letters, which further outline that:</p>
<ul>
    <li>PNBs and RFA are integral to precision-based pain management and have decades of demonstrated clinical use across multiple specialties, including Pain Medicine, PM&amp;R, Anesthesiology, Neurology, and Radiology.</li>
    <li>The proposed LCDs disregard multisociety guidelines and peer-reviewed literature confirming the effectiveness of these therapies for conditions such as occipital neuralgia, complex regional pain syndrome (CRPS), and knee osteoarthritis.</li>
    <li>Removing coverage for these procedures would contradict CMS’s own opioid reduction and value-based care initiatives, likely leading to increased reliance on opioid medications, costly surgeries, and diminished quality of life for Medicare beneficiaries
        living with chronic pain.</li>
</ul>
<p>Our societies stand ready to engage directly with the MAC medical directors to ensure that any future LCDs accurately reflect the best available evidence and preserve access to effective, non-opioid pain management options.</p>

<description><![CDATA[<p><strong><img alt="" src="https://cdn.ymaws.com/www.ipsismed.org/resource/resmgr/advocacy/25/10-PNS-BCBS-s.jpg" style="width: 95%; height: 95%;" /><br /><br /></strong>IPSIS has convened the <a href="https://www.ipsismed.org/page/MPW" target="_blank">Multisociety Pain Workgroup</a> (MPW) to call on Blue Cross Blue Shield (BCBS) plans to cover Peripheral Nerve Stimulation (PNS) for chronic pain.<br /><br />In their <a href="https://cdn.ymaws.com/www.ipsismed.org/resource/resmgr/advocacy/25/10-pns-bcbs.pdf" target="_blank">letter</a>, the MPW societies oppose the BCBS draft policies that deem PNS as experimental, investigational, and/or unproven and highlight:</p><ul><li>the robust PNS clinical data,</li><li>FDA clearance, and</li><li>Medicare national and local coverage policies demonstrating that PNS is an evidence-based therapy for neuropathic pain.</li></ul><p>The plans involved include: BCBS of Illinois, Montana, New Mexico, Oklahoma, and Texas. Their stance fails to reflect the substantial clinical evidence supporting PNS effectiveness. IPSIS is determined to ensure that patients can continue to have appropriate access to these valuable procedures.<br /></p>

<title>IPSIS Seeks Physician Waiver From New $100K Visa Fee</title>

<description><![CDATA[<p><img alt="" src="https://cdn.ymaws.com/www.ipsismed.org/resource/resmgr/advocacy/25/10-H1B-s.jpg" style="width: 95%; height: 95%;" /><br /><br />IPSIS joined the American Medical Association and 50 other national medical societies in urging the Department of Homeland Security to exempt physicians from the newly proclaimed $100,000 fee on H-1B visa applications.<br /><br />In a September 25, 2025, <a href="https://www.ipsismed.org/resource/resmgr/advocacy/25/10-H1B.pdf" target="_blank">letter</a>, Homeland Security Secretary Noem was urged to exercise her exemption powers to explicitly exclude all physicians — including residents, fellows, researchers, and those in non-clinical roles — given their essential role in addressing care shortages.<br /><br />With a projected shortfall of nearly 86,000 physicians by 2036 and 23% of U.S. physicians being foreign-trained, physicians with H-1B visas are vital to maintaining patient access to high-quality care, especially in rural and underserved communities. IPSIS is committed to protecting patient access to pain and spine care.</p>
